Request form for Birth, Marriage or Death Certified Copies  **This is a 3-Column Form so make sure you fill out the correct column (Birth, Death or Marriage) – You will need to send a copy of your driver’s license with this form and also a check made payable to the Town of Windsor in the amount of $15.00 for 1 copy.  If you wish more than 1 copy it is $15.00 for the first and $6.00 for each additional copy.)
